The training also provides information on vaccine, vaccine communication, and preparation for future outbreaks with infection prevention and control measures for community level. The project is initially focused in Bagmati Province where the the burden of COVID 19 infections is the highest, and will likely have a high burden of post-COVID complications. The training was developed from Brown University faculty and is highly evidence based, and similar trainings have been conducted at pilot in Gulmi and Dang already.
The goal of this training is to rapidly deploy a 3 module training targeted at building the capacity of FCHVs to help Nepali community be prepared against post-COVID complications, as well as prepare for future outbreaks. The topic in training will include community level IPC, keys to home care of COVID 19 and post-COVID syndrome, recognition of severe illness, vaccine background and communication, guidance on local resources, and mental health. The training uses guidelines and protocols from modules, and Nepal's Ministry of Health and Population, and utilizes videos, case scenarios and interactive question/answer. As of January 19, 2022, SarsCoV 2 continues to be in community transmission in Nepal, with the highest caseload seen in Bagmati Province. Only 51% of the total Nepali population has been fully vaccinated. With the case rise of Omicron and high per capita infeciton rate during Delta wave, post COVID complications will be a public health challenge in Nepal. Burden of post COVID syndrome on top of pre-existing mental health epidemic in the country will be a challenge for the health system in Nepal. Female Community Health Volunteers (FCHVs) are an instrumental part of healthcare in Nepal and are the last mile of health information delivery across Nepal. A recent survey of 593 FCHV representing all 7 provinces and 222 rural municipalities found that FCHVs (~89% of those surveyed) have not received training for COVID-19, vaccinations or post COVID. They are lacking training in basic COVID-19 concepts including home care and reasons to seek higher levels of care. FCHVs reported gaps in knowledge of basic COVID 19 concepts including guidelines for home care and reasons to seek higher levels of care. Survey showed gaps in knowledge on indication and contra-indications for vaccines. Majority of FCHVs demonstrated willingness to be trained and to participate in local COVID 19 efforts if they are well trained. FCHVs can be instrumental in public health fight in post-COVID world. For this, proper training to equip them is vital. This program seeks to do exactly that. |

HAPSA Nepal started as a public health education non-profit organization promoting hand hygiene. Since the Nepal earthquake, HAPSA Nepal has focused on community health advancement in Nepal. Since 2015, HAPSA Nepal has worked to rebuild health centers affected by earthquake through a innovative community and government partnership. During COVID 19, HAPSA Nepal worked closely with COVID Alliance Nepal to lead training of frontline workers, and set up of isolation centers. HAPSA Nepal also supported 30 municipalities with Home Isolation Kits to support those staying at home. Currently, HAPSA Nepal has led Female Community Health Workers (FCHVs) training for Omicron mitigation, and has already conducted multiple trainings. HAPSA Nepal is committed to equip rural communities to face the challenges of post COVID by building capacity of FCHVs. |
